North Carolina 4-H is an educational program of North CarolinaState University and North Carolina Agricultural and Technical State University and is part of the National 4-H organization. All divisions except Cloverbuds require a NC Substitute W-9 form to be submitted when you register online or by mailed in paper entry form.

Exhibitors who choose NOT to disclose their full Social Security Number at the time of registration, even if they have submitted an entry in the past, forfeit any and all premium monies they may be entitled to.
